Help for Your Patients' Families Between Visits

When a family faces a new diagnosis or a hospital stay, we help them sort out what's weighing on them most, what's working, and what they need. Then we map out a clear plan for what comes next. We work alongside the care you provide.

Help Between Visits

Between visits, families juggle specialists, home services, and paperwork. We help them get organized. On our Care Management plans, a care coordinator works directly with their other providers.

✉

Updates When Your Patient Agrees

With your patient's written permission, your office gets short, plain-language updates on how things are going at home.

Why Ellipses?

One Point of Contact for the Whole Family

We help families make sense of hard moments and map out what comes next. We work with families facing serious injury or illness, chronic and degenerative conditions, aging, and terminal illness. Ellipses is a service of Neuro Community Care, LLC, built on its work with brain injury and neurological conditions.

◐

Independent

We're self-pay and work for the family, so care isn't rushed, capped, or dictated by a policy.

⚡

Fast. Hands-On.

In most cases, we can begin within 24 hours. A dedicated care coordinator learns what the family is facing and stays involved as long as they want support.

✦

A Plan in Writing

Every plan includes a Domain Assessment built on the social determinants of health. It scores nine areas of a family's life, from medical care to money, home safety, and support, and leads to a written Care Map the family can view anytime in our secure client portal. It has interactive checklists for this week, this month, and the next 3 months, and on our monthly plans we re-evaluate both every 90 days.
