A family's story

Dana's husband, Chris, has lived with MS for years. As his symptoms changed, so did Dana's life. She worked full time, then spent her evenings on hold with insurance, rebooking physical therapy, and keeping track of three specialists. She was exhausted, and she felt guilty for being exhausted.

Dana booked a free call. She knew she couldn't manage the providers herself anymore, so she chose Coordinated Care Management and became our Care Partner. The Domain Assessment looked at Chris's care and at how Dana was holding up, too. Their Care Map listed respite options, next steps on an insurance denial, and who to call on bad days while Dana was at work.

Their coordinator now oversees Chris's neurology follow-ups, physical therapy and home health, handles the scheduling, and meets with Dana each month. Dana still worries sometimes. But Friday nights are theirs again.

This is a composite story. It combines common experiences of the families we serve. Names and details are made up.
