A plan for hard days

With Coordinated or Comprehensive Care Management, we build a crisis plan so everyone knows what to do.

A family's story

Linda's son, Marcus, was 24 when a car crash left him with a brain injury. After months in rehab, he came home with a stack of discharge papers and no clear plan. Linda had a neurologist, two therapists, a home health agency and a personal injury attorney all asking her for different things.

With this many moving parts, Linda chose Comprehensive Care Management and became our Care Partner. The Domain Assessment covered Marcus's medical care, memory and mood, and the legal questions ahead. His Care Map put follow-up appointments first, listed the records his attorney needed, and named who Linda could call on hard days.

With Marcus's permission, their coordinator now meets with his providers, works with his attorney's office, and has a plan ready if things get worse. Linda isn't the go-between anymore. She gets to be his mom again.

This is a composite story. It combines common experiences of the families we serve. Names and details are made up.
