Care Coordination for Complex Medical Needs

Care coordination is the deliberate organization of care across providers, services, and time. For families managing complex medical conditions, complex care rarely happens on its own. Doctors, specialists, and other providers each manage their piece. No one manages the whole picture. Ellipses acts as the central point of contact across all of it, advocating for patients, managing the logistics, and making sure the right supports are in place at every stage.

This page covers what care coordination services include, who they are for, how Ellipses structures its support, and how to get started.

Care Coordination: Common Questions

Is this the same as case management?

Our focus is family-centered navigation and alignment: clarifying goals, reducing fragmentation, and helping you access the right services locally.

Do you talk to my doctors?

With your consent, yes. We communicate with providers and payers to align the plan, support authorizations, and reduce delays.

Do you need a license or degree to be a Care Coordinator?

Requirements vary by organization and setting. At Ellipses, Care Coordinators are trained professionals with backgrounds in health care, social services, or related fields. They work alongside licensed clinicians but are not themselves required to hold clinical licensure. The combination of trained coordination and licensed clinical care, operating as a coordinated team under one plan, is the model Ellipses is built on.

When Care Coordination Makes the Difference

For families managing complex medical conditions, the difference between coordinated care and fragmented care is not subtle. It shows up in whether medications are reconciled, whether providers are working from the same plan, whether the family understands what is happening and what comes next, and whether the person doing the caregiving has any support of their own.

Care coordination services don't replace the clinical team. They make the clinical team work. That is what Ellipses is here to do.
