How It Works

From overwhelmed to organized, one step at a time

When someone you love is sick or hurt, there is too much to keep track of. Doctors, bills, forms, schedules. We help you sort it out. Here is what working with us looks like.

Who's who

Two terms you will hear us use.

Care Recipient

The person who needs care.

Care Partner

The caregiver who works with us as our main contact. Often a spouse, adult child or parent.

Both

Managing your own care? Then you are both our Care Partner and our Care Recipient.

Your path, step by step

Every family's situation is different. The path to a clear plan is the same.

1

A free 15-minute call

You fill out a short form and pick a time. A care advisor meets with you by video on Microsoft Teams. You tell us what is going on. We listen and answer your questions. There is no cost and no pressure.

2

Choose your plan

After your call, you choose the plan that fits your family. Every plan includes the Domain Assessment and a written Care Map.

Care Roadmap

A one-time plan: your Domain Assessment and a written Care Map.

Care Copilot

Monthly check-ins and research to help you carry out your Care Map. Starts after the Care Roadmap.

Coordinated Care Management

Everything in the Care Roadmap, plus a coordinator who works directly with your providers, handles scheduling, and meets with you each month.

Comprehensive Care Management

Everything in Coordinated Care Management, plus no cap on services, meetings with your providers, crisis planning and attorney collaboration.

Your first session: the Domain Assessment

Your plan starts with a first session with your care coordinator. Together, we complete our Domain Assessment. It looks at nine key areas of life. Each area gets a score. Together, the scores show how complex your situation is and where help is needed most.

Medical care & rehab
Daily living & mobility
Memory, thinking & behavior
Emotional & mental health
Caregiver support
Money & insurance
Legal planning & decisions
Home safety
Social life & community
Together, the scores show how complex things are and where to start

Your Care Map

Next, we build your Care Map. It is your family's plan in writing, and you can view it anytime in your secure client portal.

5

Adjust as your needs change

Families' needs change, and your plan can too. You can upgrade or downgrade to any plan at any time with 30 days' notice. We check your Domain Assessment again over time to see what has changed.

6

Simple, transparent billing

Monthly plans come together in one bill. We are self-pay, so your care is not rushed or limited by insurance rules. There are no long-term contracts and no hidden fees.
