Managing Multiple Conditions and Mental Health

People managing multiple chronic conditions face compounding complexity. Treatments interact, medications affect one another, and providers give conflicting guidance. We coordinate across the full care team to keep everything aligned. We also connect clients with licensed mental health professionals who use strategies including cognitive behavioral therapy to help manage chronic pain, depression, and anxiety, and we connect people to free support groups, virtual and in person, for conditions including diabetes, arthritis, heart disease, asthma, hypertension, and fibromyalgia.

Flare, Progression, or Crisis

Flare Response, Financial Assistance, and Caregiver Support

When a chronic condition progresses or a major flare disrupts daily life, we support clients in requesting adjustments to their care plan, help implement needed modifications, and coordinate provider responses. We also help clients access financial assistance programs, including the PAN Foundation and disease-specific nonprofits, for eligible patients facing the costs of ongoing treatments and medications. Parents, spouses, and other family members supporting loved ones with chronic illness carry real weight too. We connect caregivers with support groups and counseling so they can keep going.

Chronic illness care involves multiple providers, medications, and insurance companies, none of them coordinated by default. We act as the primary single point of contact, keeping providers aligned and connecting clients with the programs most people never find: the Center for Chronic Illness, NCOA self-management programs, and condition-specific resources that reduce the burden of managing ongoing health challenges alone.

For individuals and families outside the Triangle, we work virtually and nationwide.
